Intermediate Care

What is Intermediate Care?

Intermediate or after hospital care is provided by services on a short term basis, at home or in a residential setting to those who need some degree of rehabilitation, convalescence, or recuperation.

 

If a family member or friend who you know has been in hospital or had an illness or fall, temporary care to help you get back to normal is the best option to take whilst staying in our care home. 

Short-term care after being discharged from hospital is sometimes called intermediate care, or aftercare. Reablement is a type of care that helps you relearn how to do daily activities.

Intermediate Care at The Village Care Home

After leaving hospital, care at a care home can help you recover from an illness or an operation. 

​

Hospital staff will assess your health and help decided whether you need further help to ensure you fully recover. If they think care is the suited option post hospital, they will arrange care before you leave the hospital.

​
